Step 1: Verify Your Coverage
Your nutrition appointments may be covered if you have any of the following insurance plans. It is your responsibility to contact a member services representative to verify coverage and whether or not you have a co-pay for a specialist visit. The phone number is on the back of your insurance card.

Step 2: Schedule Your Appointment
Once you’ve found out that your insurance will cover the visit, book your initial in-person consultation. Unfortunately, video chats are not covered by insurance in our area at this point.
Step 3: Fill out your Initial Consultation Forms
You will receive a confirmation email for your appointment, along with a link to fill out initial paperwork forms online. Please be sure to complete these forms, which should take no more than 10-15 minutes, as soon as possible in preparation for your visit.

Frequently asked questions about insurance coverage
Q: Do you accept insurance coverage?
A: Yes, currently we are contracted with the above healthcare programs.
Currently most local insurances cover in-person, phone and video-chat nutrition counseling services. Currently It is your responsibility to contact a member services representative to verify coverage. The phone number is on the back of your insurance card.
Q: What is the cost of nutrition counseling?
A: If your insurance covers nutrition counseling, there is no cost to you, unless your insurance plan requires a co-payment for this service. In that case, you will be responsible for payment of that co-pay at each visit.
The private pay rate for an initial consultation is $100 and a follow-up consultation is $50. This is considered a discounted rate for private pay clients, those who pay out-of-pocket at the time of service. Private payment is paid with a credit card at the time you book your appointment. If we submit a claim for nutrition counseling and your claim is denied, you will be responsible to pay the full insurance rate.
Q: Will I need a referral from my doctor?
A: No. You can schedule nutrition counseling without a doctor’s referral.
Q: How many times can I see a nutritionist?
A: That depends on your plan. Because there are many different plans, it is your responsibility to call the member services number on the back of your card to verify your coverage for nutrition counseling and to see if there is a limit to the number of nutrition counseling sessions allowed.
Q: Can I use my HSA or FSA account to pay for products and services at Active Nutrition?
A: We cannot guarantee our products and services are allowable under your HSA or FSA account. Contact your HSA or FSA account manager for more information.
Q: Does insurance cover cooking parties, demos and classes?
A: No, unfortunately these offerings are not covered by insurance.
